[How to apply]
① After changing your registered address to your current address on "My Page", select the "Go to confirmation screen" button at the bottom of this reception form.
②Select the "Submit" button on the confirmation screen to complete your application.
※Items to check when changing your My Page※
・If you are not receiving your package even though your address is correct, please check that you have entered the building name.
・If the name on your home nameplate is different from the addressee's name, the package will be returned due to "unknown addressee." If this applies to you, please register "●● (name on nameplate)" at the end of your address.
[Regarding defective products]
In the unlikely event that you receive a defective product, we will exchange it.
Please contact us using the inquiry form during the period for accepting defective products listed below.

* [Renewal Benefits]Those who complete the renewal procedure between 00:00 on Thursday, September 1, 2022 and 23:59 on Wednesday, February 1, 2023 will receive a renewal benefit enclosed.
*The re-delivery is scheduled for early April 2023. We will notify you again when the items are shipped, so please wait for a while until they arrive. (As we will be re-delivering items all at once to members who have contacted us via the non-delivery reception form, we will not be able to inform you of the re-delivery status or respond to your inquiries individually.)
* Items will be delivered to the address you specify. If the item cannot be delivered due to reasons such as the mailbox being too small, it will be returned and a notice of absence will be posted, so please arrange for redelivery yourself.
